with questions. Details: Head to the claims administrator website. Click on the "File a Claim" option on the left. The website walks you through the process. If you've already received a card in the mail with your claimant number, make sure to fill it in. If you haven't received the card in the mail that's OK. Continue with the process and fill out the information to the best of your ability. MAKE SURE TO PRINT OUT A COPY OF YOUR COMPLETED CLAIM FORM! Keep in mind that unless you are sending in proof of your claim, you don't have to submit a paper claim. If you are worried about getting paid the full amount of your claim our opinion is that you should send in a paper copy of your claim, along with the proof to the address above. If you don't do that, and Verizon Wireless can't "locate" your ETF charge they will likely only give you the default amount of $25. We (and I'm sure you) don't want to see that happen.If you've moved, or canceled your service and were billed the ETF at a previous address, again, in our opinion, your best bet is to send in the paper claim with the proof of the claim (if you have it.) That's it! With the settlement going before the judge on October 24th, there's a SLIM chance you'll get a check in time to go towards your Chirstmas / Hanaukka / Kwanzaa credit card card bill you receive in January. It's more likely the check won't arrive until close to the middle of next year, but, you never know! Verizon Wireless settled the ETF class action lawsuit in California and extended the settlement to cover the entire U.S. If you were charged an ETF by Verizon Wireless between July 23rd, 1999 and August 10th, 2008 than you should submit a claim.VERY SIMPLE! What are some of the interesting points of this settlement? First, if Verizon Wireless can't locate the proof that they charged you an ETF (keep in mind that this is going back to 1999 so the records may be a little sketchy) then they will STILL give you $25 (or less if the settlement fund is almost tapped out but NOT less then $5.) Yay. That's pretty cool in our opinion. No, not the $5 part, the part about no proof of the charge still netting you up to $25. All you have to do is file the electronic claim, put in your info, and you'll get a check for $25 (or whatever is left in the settlement fund.) THAT'S cool. So, head over to the Settlement Administrator website and stake your claim. I did, shouldn't you? Just received the post card in my P.O. Box this week. It would have been nice if you all could have sent the claim form without us having to go the website for it. I really needed to take advantage of this opportunity because Verizon charged me an early termination fee because they cancelled me while I was still making payments on my account. My payments were not enough they said and I had been a customer with Verizon for over 10 years and subscribed to three lines with them.
This process should have been handled a little better. Thanks

Top Class Actions is unable to send claim forms to those who request them. Those who need claim forms need to go to the settlement administrator website and fill them out and print them there. I apologize that you're having difficulty doing so. If you are unable to go to the Settlement Administrator website I would recommend calling them and seeing if they can send you a form in the mail. For the Verizon case, the settlement administrator doesn't have a phone number listed. I hope this helps! Keep in mind, that the claim form has to be postmarked by October 14th, so you still have the opportunity to head to a Kinko's to print out the claim form or ask a friend or neighbor to print it out for you.
