My sprint service was canceled 2 years ago. I have documentation for the account number and phone number but sprint claims they have no record for that account so I cannot verify early termination fees (nor according to what they've told me can they!).

I cannot access the form to submit a claim for this class action lawsuit. Does anyone have a document that can be sent as an attachment?
