I also experienced going to the "Settlement Administrator" page and clinking on "File Claim" and only receiving a blank page.

If you are usine Internet Explorer 6.0 you are unable to see some things on the internet.  Resolution to this problem is to either upgrade Internet Explorer or if you are like me who needs to keep using I.E. 6.0 for other reaseons, then just hop on over to Mozilla and the form will populate.  Hope this helps those who are having promblems with the "Settlement Administrator" page.
