Details: Head over to the Settlement Administrator Website and print out the Consumer claim form. It’s actually a REALLY easy claim process. You just fill out your name and street address and list how many split tablets you had. If you had 5 or less split tablets, you’ll get a check for $50, no proof of purchase, receipt or other verification needed. If you had MORE than five split tablets, you can claim up to 15, you’ll receive $50 for the first five, PLUS $10 for each additional broken tablet, up to $150 total. If you state you had more than five split tablets you MUST include verification that you paid for or were prescribed the tablets during the acceptable time frame (April 1, 2002 and March 4, 2005) in the envelope with your claim form. You need to submit one (and only one) of the items below as proof.

(1) a written prescription for Paxil CR® between April 1, 2002 and March 4, 2005;
(2) a receipt, canceled check, or credit card statement that shows you paid (at least in part) for Paxil CR® between April 1, 2002
and March 4, 2005;
(3) an EOB (explanation of benefits) form that shows that you paid or were obligated to make a percentage co-payment for Paxil
CR® between April 1, 2002 and March 4, 2005; or
(4) a letter from your physician stating that he or she prescribed and that you paid for or were obligated to pay at least in part for
Paxil CR® between April 1, 2002 and March 4, 2005.

If you do so, you can get a much bigger check. We think it’d be worth going to your Doctor and asking them for a letter stating you took the Paxil CR between April 1, 2002 and March 4, 2005. They’ll mail or fax you a copy for free, so why not ask? That could get you an extra $100 so in our opinion, that’s worth a few minutes of work. Make sure to sign, print your name AND date your claim form. If you forget any of those items your claim will likely be denied. Put your claim form and any necessary proof into an properly addressed envelope and drop it in the mail. You’ll likely get your check at the end of 2009 or early 2010. That’s it! Now that’s an easy claim to file.

TCA Staff Tidbit: This class action lawsuit settlement claim ended up being VERY easy to file. Just follow the instructions below and you’re on your way to $50 – $150. That’s GREAT! I’d also applaud the attorneys working this case by granting a long claim window. Too often we see claim windows of sixty days which just doesn’t give the affected class much time to get their claim in the mail. I’m also happy to see that anyone who took the drug (and received broken tablets) can claim their $50 without any proof of purchase. That makes it easy on the affected class which is a GOOD thing. So, I’d still recommend you call your Doc and get that letter faxed to you stating you took the medication from 4/1/2002 – 3/4/2005 if you received 15 (or more) broken tablets. Couldn’t you use the extra $100 you’d get? Isn’t it worth the 15 minutes of hassle (call your Doc, wait on hold, wait for the letter, call your Doc back asking for the letter again, wait on hold, eventually receive the letter and send in your claim.) Heck, if it only takes you 15 minutes you are actually earning $600 an hour! Now wasn’t that a nice feeling? If you can, MAKE YOUR CLAIM! This is a SUPER EASY CLAIM and MILLIONS of you took the pill during this time, so I hope at least a few hundred thousand claim their share.
